Fly Again Acupuncture COVID-19 Policy
You must fill out the COVID-19 Consent Form on the patient portal before making an appointment at Fly Again Acupuncture. You only need to complete this form once. Please check each box to certify that you have read and understand each statement and sign the bottom of the form. These policies apply to every appointment until further notice.
Masks in the office are now optional for both patients and staff. You may choose to wear a mask, or not, without judgement or questions at any time. We ask that you continue to follow the COVID monitoring guidlines and reschedule if you have symptoms or have been exposed. We also ask that you DO wear a mask if you are coming to the office for treatment of a non-COVID cold, pneumonia, cough etc.
As always, your safety is our highest priority and we will continue to sanitize surfaces and treatment rooms in between patients and keep our air purification system running. If you would prefer that we wear masks during any treatment session please don't hesitate to ask. We understand that people have newborns, visits with elderly relatives, travel plans and a hundred other reasons to want to avoid catching a bug.
If you have been exposed to a person with a confirmed case of COVID-19 within the past 5 days you may not attend an appointment at Fly Again Acupuncture within 5 days of your exposure or until you have a negative COVID-19 test.
In the last 48 hours, if you have experienced:
fever over 100.5°F/38°C
shortness of breath or difficulty breathing
new loss of sense of smell or change in taste
you may not attend an appointment at Fly Again Acupuncture. Unfortunately, we cannot treat you at this time. You may reschedule after 14 days or when you have had a negative COVID-19 test result.
In the 48 hours before your appointment if you have experienced three or more of the following symptoms you may not attend an appointment at Fly Again Acupuncture. Unfortunately, we cannot treat you at this time. We will be happy to offer a telehealth consult for herbal medicine, and/or refer you to an appropriate facility.
New nasal congestion or runny nose
loss of sense of smell
New body or muscle aches
New changes in skin (rash, skin discoloration, discoloration of toes)
New chills, feeling cold, or shivering
If you are aged 65 years or older, have chronic lung disease or moderate to severe asthma, have a serious heart condition, are immuno-compromised including cancer treatment, or have poorly controlled underlying medical conditions like diabetes, renal failure or liver disease you are at high risk for COVID-19. You should follow all “Stay at Home” orders and use extra precautions at all times. The only way to minimize your risk of COVID-19 Infection is to stay at home.
When you arrive, please come in and have a seat in the waiting area. Someone will come get you when we are ready for you.
As always, linens will be laundered after each use. Treatment room surfaces will also be sanitized with alcohol after each patient.
If you test positive for COVID-19 and have had a recent appointment at Fly Again Acupuncture you are requested to notify the office immediately.
If a patient recently seen at Fly Again Acupuncture or any member of the staff tests positive for COVID-19 the clinic will follow the guidance of the local health department and close if necessary. Patients who were seen in the office and may have a risk of exposure will be notified of the possible exposure via phone and email.
Cancellation fees will be waived for anyone who needs to cancel an appointment on short notice due to COVID-19 illness or exposure. No show fees still stand, please call if you need to cancel your appointment.
Even with the above precautions that there is still a risk of COVID-19 exposure whenever you enter a public space. You understand and accept this risk by attending an appointment at Fly Again Acupuncture.
