PAYMENT POLICY

Acupuncture and related services are provided and billed by Fly Again Acupuncture PLLC. Services administered must be paid for at the time of treatment.

Fly Again Acupuncture will work with you to get your treatments paid for by your insurance. Call your insurance company to see if acupuncture is covered. We can provide you with a form that you can submit to your insurance company to instruct them to send reimbursements directly to you.

Please be aware that with any health plan, there may be deductibles and copayments for which you could be responsible and it is not guaranteed that your acupuncture treatment will be covered.

SERVICE FEES

(Only if paid in full at time of service, other charges may apply if your insurance is billed)

  • First Acupuncture Appointment - $120
    (includes a comprehensive health interview)

  • Follow up Acupuncture Appointments - $85

  • First NAET Appointment - $155

  • (includes a comprehensive health interview and NAET reference materials)

  • Follow up NAET Appointments - $95-$105
    (Price depends on extra allergy testing done that day)

  • Herbal Consult Only, First Appointment - $100
    (includes a comprehensive health interview)

    • Patients are also responsible for the cost of the herbal formula recommended to them. The cost of an herbal formula varies on a case by case basis.

  • Herbal Consult Only, Follow Up Appointment - $55

    • ​Patients are also responsible for the cost of the herbal formula recommended to them. The cost of an herbal formula varies on a case by case basis.

  • Services such as cupping, tui na, gua sha, etc. may be included in your acupuncture treatment at the practitioner's discretion at no extra charge. 

  • Emotion Code Sessions - $70

    • ​​Includes a review of main concerns, kinesiology, and magnetic emotional release.

  • Emotion Code Pet Session - $70

    • ​​Includes an interview of main concerns, kinesiology, and magnetic emotional release for your pet.

PAYMENT OPTIONS

We accept payment via

  • Cash

  • Check

  • Credit/debit card 

  • Heath Savings or Wellness Card

  • MVP Insurance

  • CDPHP Insurance

  • Optum Insurance

  • United Healthcare including the Empire Plan

Patients successfully get direct reimbursement from 

  • Empire BlueCross/BlueShield

  • Anthem

 

Fly Again Acupuncture will work with you and your insurance. Call your insurance company to see if your plan covers acupuncture for you and your family. If your insurance covers acupuncture and we are not in network, we will be happy to provide you with a receipt for reimbursement. 

 

HSA/FSA cards are now accepted through square. 

If for some reason your card does not work, acupuncture treatments are still eligible for coverage through your health savings account. However, you will need to bring another form of payment to your appointments. We will set you up with the receipts to be reimbursed.

OFFICE POLICIES

Cancellation Policy

If you need to cancel an appointment please do so at least 24 hours in advance. If you cancel less than 24 hours before your appointment you may be charged a $50 fee for a late cancellation. If you need to cancel your appointment due to inclement weather please do so at least 2 hours prior to your appointment via call or text (518) 855-1455

 

Late for your appointment

If you are more than 15 (fifteen) minutes late for your appointment you may not be treated or you may receive an abbreviated treatment within the time constraints of your originally scheduled appointment.

If your appointment is rescheduled you will be responsible for the $50 late cancellation fee. 

 

Missed appointment

If you miss an appointment without calling to notify the office at least 24 hours in advance you will be charged the full cost of your missed appointment. You will receive an invoice via email. Or we will charge your card on file. 

 

Bounced Checks
There will be a $30 fee for each bounced check.

Fly Again Acupuncture currently, participates in CDPHP, United Healthcare and MVP Insurance plans. If you do not have one of the aforementioned plans, we can provide you with a form that you can submit to your insurance company to allow them to send any reimbursements directly to you.

Please be aware that with any health plan, there may be deductibles and copayments for which you could be responsible and it is not guaranteed that your acupuncture treatment will be covered. You are responsible for paying for any service that is denied or otherwise considered not covered or not reasonable or necessary by your insurer. You will be notified of any copayments or other fees that you are responsible for. If these fees remain unpaid 180 days after your date of service they will be sent to a collection agency.
